Dear Community Leader:
Cook Children’s needs your help with a confidential survey about children’s health in our area. Every three years, we conduct a Community Health Needs Assessment (CHNA) to identify the children’s health needs in our community and develop solutions with community partners to address the most important issues. We need information from community leaders in the following counties (Collin, Denton, Grayson, Hood, Johnson, Parker, Tarrant, and Wise).

In addition to this survey, the CHNA includes a household survey of parents and caregivers of children ages 0-17, qualitative data from families, and a review of facts from government and other health organizations. The unique perspective that you provide as a community leader helps us integrate all of this important information, and develop a plan to collaboratively address the identified health priorities. This information also enables us to track our progress in addressing health needs over time.

Your opinions and experiences are important, and can help us determine community needs to better serve our families. We invite you to take a few minutes to complete this private survey. It is a simple way that you can give back to your community and make our region an even better place to live.

ETC Institute is a research firm that works with communities all across the country. ETC will compile the survey results for Cook Children’s and we will begin sharing the results with the community in the coming months.
